Licensed Practical Nurse (Program Coordinator)

Job Description

Under the general supervision of the Program Manager, the Licensed Practical Nurse (Program Coordinator) is the first point of contact for the Diabetic Foot Screening. They will be responsible and be accountable for key activities associated with the screening programs by gathering information, scheduling and coordinating imaging exams, providing instruction and/or direction to patients as necessary both on the phone and in person, preparing all documentation and records with accuracy and all necessary information including but not limited to medical referrals, lab results, prior authorizations, demographics and insurance information to ensure appropriate billing.


ESSENTIAL FUNCTIONS:

  • Coordinates and leads the functions of scheduling and screening for the Diabetic Foot Screening Program.

  • Coordinates and hosts educational and screening booths at health fairs.

  • Conducts Diabetic Foot Screenings on eligible program candidates.

  • Maintains participant information and ensures follow-up studies are ordered and scheduled.

  • Ensures patient correspondence is prepared and sent in a timely manner

  • Ensures compliance with program regulations.

  • Accurately tracks and documents communication with patients; calls and sends letters to patients who are due for tests and follows patients until all annual screening are complete.

  • Monitors and reviews schedule seventy-two hours (72) prior ensuring accuracy, maximization of clinical operational hours and schedule efficiency.

  • Monitors and reviews orders for Diabetic Foot Screening ensuring appropriate scheduling and turnaround time are executed.

  • Provides culturally appropriate support and information for patients and their families through follow up phone calls and makes reminder calls as needed.

  • Completes monthly audits of patients for several quality health indicators and other quality improvement activities as needed.

  • Continuously expands knowledge and understanding of community resources, services and programs provided; attends outreach/education events in the community and conducts health education workshops at clinic and in the community.

  • Obtains and enters new patient demographics; updates patient information, as necessary, in the patient electronic records to maintain accuracy for billing purposes.

  • Obtains all required information/documents for scheduled procedure; may require verification by contacting insurance companies, laboratories or referring physicians; ensures lab results are cleared and acceptable for scheduling confirmation.

  • Maintains strictest confidentiality; adheres to all HIPAA guidelines/regulations.

Requirements

EDUCATION:

  • High school or equivalent (Preferred)

  • Must possess license as a Licensed Practical Nurse or Registered Nurse.
