Patient Forms
To ensure we have the most updated information, each patient of GRC must complete the patient registration form. Complete and accurate registration forms ensures that you do not get incorrectly billed.
A Medical Records Release Form is used to request that a health care provider (physician, dentist, hospital, chiropractor, etc.) release a patient's medical records to Guam Radiology Consultants. Having your prior records allows our radiologists a better understanding of your medical history to address your individual needs.
The DOD2870 form is a Department of Defense document used for the authorization of the release of medical information. This form allows a patient to grant permission for their healthcare provider to share specific medical records with designated individuals or entities. The form ensures that the disclosure of medical information complies with privacy regulations, safeguarding the patient's rights while facilitating necessary communication for their care or other legitimate purposes. Completing the DOD2870 form involves detailing the type of information to be released, the recipients, and the duration of the authorization.
